Electronic Background Check
All AGP volunteers must complete and pass a background check. Background checks are to be completed online by clicking the button below. Please contact us at adaptivegolfprogram@gmail.com if you are in need of the password. If you would like to donate the cost of running your background check, select “Applicant Pay Background Check.” AGP will happily cover the cost by selecting “Company Pay Background Check.”
Volunteer waiver
Please fill out the Volunteer Waiver below BEFORE you sign up to volunteer for the program sessions. You will not be able to participate in the program if the Volunteer Waiver has not been filled out. If you have already filled out the Volunteer Waiver please proceed to VOLUNTEER SIGN UP.
